By accepting your athletes spot on the Tualatin Youth Track Team you acknowledge and understand that participation in athletic activities involves inherent risks, including but not limited to physical injury and exposure to communicable diseases (e.g., MRSA, influenza,
COVID-19). I, on behalf of myself, my heirs, and my personal representatives, hereby release, waive, and hold harmless TuYTC, its coaches, volunteers, and affiliates from any claims, liabilities, or damages that may arise from my child’s participation in TuYTC activities.
Medical Certification & Health Information
By accepting your athletes spot on the Tualatin Youth Track Team you certify that, to the best of your knowledge, your child has no medical or psychological condition that would prohibit or restrict their participation in TuYTC activities. I understand that it is my

In the event of a medical emergency during TuYTC activities, I grant permission for any TuYTC
representative to seek necessary medical care for my child. I understand that:
I certify that I am the parent/legal guardian of the athlete(s) listed below, that I am 18 years of age or older, and that I have read, understood, and agreed to the terms outlined in this waiver & consent form. I also acknowledge my responsibility to provide all required information to the
TuYTC team, coaches, and my child's physician as necessary.
- Safety is paramount in our program. Any athlete or associated guardian who poses a risk of physical or emotional harm to themselves or others—including TuYTC members, athletes from other teams, officials, or bystanders—will face appropriate consequences. This may include dismissal from the team without refund.
- Athlete should also have access to appropriate clothing, protection (including rain and sun), water, and nutrition.
- Athletes are expected to actively listen, engage, and participate in all TuYTC activities. If an athlete’s behavior consistently causes distractions or diminishes the experience of their teammates, alternative arrangements may be considered, including dismissal from the team if necessary.
- Our program requires consistent coordination and support to ensure a safe and positive experience for all athletes. Volunteer and coaching participation is an essential part of our community and is expected in accordance with the commitments identified prior to acceptance. Failure to fulfill these responsibilities may result in additional fee collection to support the program’s operational needs. - Coordination for meets occurs via email and the BYGA app. Using this app and responding to information requests is required for meet enrollment and participation. Failure to complete the necessary steps by the designated deadlines will result in the athlete being unable to participate in the meet.
- U8 Category Athletes: Must be picked up by an authorized adult at the designated arrival and dismissal area near the track during practices.
- Meet Supervision: Athletes must check in and out with the team at meets. No athlete should be left unattended at meets as a designated guardian must be present at all times.

- Disclose any relevant medical conditions through the data collection link that will be emailed to me.
- Discuss any significant medical risks with my child's physician and the TuYTC leadership team.
In the event of a medical emergency during TuYTC activities, I grant permission for any TuYTC
representative to seek necessary medical care for my child. I understand that:
- Medical treatment will be sought from licensed medical providers as necessary, except for basic first aid.
- I will be notified as soon as possible in the event of an emergency.
- I accept full financial responsibility for any medical expenses incurred.
